Client Care Coordinator – West Vancouver | $55,000 - $60,000 per year

Home Instead is an industry leader in providing non-medical home support to seniors around the globe. With an increased demand for our services, we are looking for a dedicated Client Care Coordinator to join our team in West Vancouver and help us enhance the lives of aging adults and their families. Please only apply if you are able to work Tuesday to Saturday.

As a Client Care Coordinator, you will perform a variety of duties to ensure the highest level of care and service for our clients. This includes consulting with potential clients and their families, developing personalized care plans, introducing Care Professionals, conducting quality assurance visits, and ensuring all aspects of client care are met. You will also promote our services, build strong relationships with healthcare providers, and advocate for seniors in our community.

Primary Responsibilities:

  • Meet with potential clients and their families to discuss their needs and provide tailored solutions in the form of a service plan.
  • Conduct service inquiries and care consultations as required, following a consultative sales process.
  • Maintain regular communication and a consistent follow-up schedule with potential clients and families who have not yet committed to services.
  • Coordinate various aspects of client care with other team members.
  • Introduce Care Professionals to clients and ensure they are fully informed of each client’s care needs and preferences.
  • Recognize opportunities to modify service plans to better support the evolving needs of clients through our quality assurance program.
  • Conduct regular quality assurance visits with clients, either in person or via phone.
  • Foster open and effective communication with clients, families, referral providers, and the internal support team.
  • Build and maintain strong relationships with existing and potential referral partners to promote business growth.
  • Maintain accurate records of all sales and prospecting activities.
  • Meet clearly defined key performance indicators (KPIs).
  • Participate in various public relations strategies.

Education/Experience Requirements:

  • A Licensed Practical Nurse (LPN) designation is preferred.
  • At least one year of experience in home care, healthcare, or a senior-related industry is required. An equivalent combination of education and work experience may be considered.
  • Must possess a valid BC driver’s license.
  • Participation in an on-call rotation to assist with urgent matters as needed.
