Post Hospital Care
After visiting the hospital, going home doesn't have to be scary! Placement or leaving their current residence shouldn't be the default option. Your loved one can safely return home with the help of our Care Team and Care Professionals working with Home Instead, thanks to a customized care plan catered to their particular needs.
Did you know that failure to follow discharge instructions or improper management of medications are primary causes of hospital readmission? In order to guarantee that your return home is successful, we collaborate closely with you and the AHS team of nurses and doctors. To plan a seamless return to your home, our Care Team will meet with you and your AHS Transition Services team.
Understanding your discharge instructions is so important! Knowing what medications you are supposed to take and when is the next most important thing to remember when you get home. Upon discharge all these instructions can be very confusing if you are elderly, need Home Care, have any memory issues/Dementia, or are just exhausted from your hospital stay. We can help by being there to listen to the discharge instructions, help you once you are home to remember to take your medications properly and help you get your strength back with exercises and getting back into your normal routine.
