Objective:

The nurse is expected to perform a variety of duties that relate to client care including taking service inquiries, care consultations with potential clients and family members, client/care professional introductions and quality assurance visits with existing clients. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. The Nurse will also perform “designated” nursing tasks as well as overseeing and supervising and training care professionals who will perform Medically Delegated Tasks.Primary Responsibilities:

· Provide nursing services, within defined scope of practice, to patients based on patient assessment and care planning procedures

· Monitor patients' progress, evaluate and document effectiveness of nursing interventions and collaborate with appropriate members of health care team

· Provide safety and health education to individuals and their families.

Work collaboratively with other members of the Client Care Team/Scheduling Team to follow up with clients to maintain and grow care hours.

Utilize Quality Assurance program to keep and grow client hours, while maintaining positive relationships with Clients.

Supervise caregivers and provide direction and management to them

Conduct training to staff as required- Personal Care, Alzheimer’s Care, Specialized Care, etc.

Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.

Work with other team members to coordinate various aspects of a client’s care.

Conduct client/care professional introductions with every new client and with every new care professional.

Create and maintain client and responsible party records documenting all quality assurance meetings.

Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.

Demonstrate open/effective and positive communication with clients, family members, the franchise owners, colleagues and care professionals, and referral providers/care providers.

Work with clients and their families on issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.

Demonstrate open and effective communication with the franchise owner, colleagues, care professionals, clients and family members.

Maintain regular attendance at the office to execute job responsibilities.

Develop Policies and Procedures regarding Medically Delegated Tasks.

Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.

Secondary Responsibilities:

· Participate as needed in all care professional meetings.

· Conduct Family Education sessions as needed.

· Perform all other functions deemed necessary.

Critical Numbers:

· Conduct the initial client/ care professional introduction for every new client as assigned.

· Conduct QA visits.

Education/Experience Requirements:

· Must maintain an updated license/permit in good standing following all provincial/state regulations and requirements.

· Must possess a valid driver’s license

Job Types: Full-time, Part-time

Pay: $30.00-$40.00 per hour

Expected hours: 25 – 40 per week
