Jake Mitten, Owner and Managing Director

Jake Mitten, Owner and Managing Director

Jake has always enjoyed taking on new challenges in both his professional and personal life, which ultimately drove him to opening a Home Instead business. Working in a fast-paced sales environment, Jake developed the skills to build effective relationships and solve problems in collaborative and creative ways. A graduate of the Bachelor of Business Administration (BBA) program at Wilfrid Laurier University, Jake is passionate about making a positive difference in peoples’ lives, and is truly thrilled he now has the opportunity to help others in the community through his work at Home Instead. He is striving to build the best home care company in Markham for both the seniors we serve and the caregivers we employ. Loyalty and patience are two of his key traits – evident in the fact he remains a lifelong fan of the Toronto Maple Leafs.

Tracy M5

Tracy Mitten, RN, Owner and Director of Client Care

A passionate nurse for over 30 years, Tracy knew from a young age she wanted a career where she could make a positive impact in peoples’ lives. Throughout her career, Tracy has worked in a variety of nursing fields, including nephrology (providing hemo and peritoneal dialysis) and chronic disease management.

Most recently, Tracy has spent the last 10 years working in a geriatric outreach program and with Behavioural Supports Ontario managing behaviours associated with dementia and other neurological conditions for individuals living in long term care. Through this experience, Tracy witnessed the challenges families face in caring for their aging loved ones first-hand, and clearly saw the need for improved private home care services. This is what led her to open an in-home care agency here in Markham. Tracy is excited to make a difference in the lives of seniors and their families and believes Home Instead’s mission aligns directly with her vision for enhancing the lives of older adults. She is a graduate from Ryerson University’s BSc. N program, which she completed part-time while raising a family of three children.

Leanne Sealey, RPN, Client Care Manager

Leanne has always enjoyed helping those in need which is what motivated her to work in the healthcare industry. She has been nursing for several years with experience both in the community providing medical care for seniors and at a children’s rehabilitation hospital in various roles. Most recently Leanne worked at that hospital as a clinic coordinator which allowed her to continue to serve patients, but in a capacity where she helped organize their plan of care and was able to spend more time with the families. She loved taking on a leadership role with patients and found the perfect opportunity to continue doing so by joining Home Instead. She was also very excited at the opportunity to serve our senior population in Markham where she grew up. As our Client Care Coordinator, Leanne is focused on ensuring our client’s receive the highest quality service. She is involved in overseeing every aspect of the client’s care including conducting the original assessment and designing the initial care plan, performing ongoing quality assurance visits, and working collaboratively with clients, families, and our Care Pros to ensure all needs are being met. What she loves most about her job is being able to come up with creative solutions to solve whatever problems her clients might be facing. When Leanne is not hard at work she enjoys spending time with family and friends, trying new restaurants, or spending time watching Netflix

Jessica Amos Staffing Coordinator

Jessica Amos, Staffing Coordinator

Providing the highest quality client service is something that Jessica takes immense pride in. It’s also an area that much of her previous work experience has focused on, including the 10 years she spent as an International Travel Consultant. In that role she was able to combine her client service mindset with her love of travel. Jessica then spent a few years working as a membership specialist with a service based organization where she managed client accounts, provided ongoing quality assurance and onboarded new clients.

After learning about Home Instead and the personalized home care service we provide our clients, she knew this was an organization she wanted to be apart of. Our slogan “To Us, It’s Personal” really resonated with her and knowing how we help seniors who need it most was a mission that excited her.

As our Staffing Coordinator, Jessica’s main priority is ensuring all our organization’s hiring needs are being met. She is proactive in recruiting, screening, hiring, training, and engaging a staff of Care Professionals to provide the best senior in-home care to our clients. As someone who enjoys taking on new challenges, Jessica also gets involved in other areas of the business including with scheduling and supporting clients.

Outside of Home Instead she spends her free time hanging out with loved ones, playing billiards, watching live sporting events, or doing yoga. She is also a huge animal lover that volunteers with Toronto Cat Rescue on top of looking after two cats of her own.

Maddy Mitten, Office Coordinator

Maddy brings enthusiasm and dedication to her role as Office Coordinator at Home Instead. With a focus on scheduling caregivers and clients, office administration, and caregiver engagement, Maddy ensures that our operations run smoothly and efficiently. Her role is pivotal in maintaining the high standards of care and service that Home Instead is known for. Maddy has always been passionate about healthcare and helping others. She holds a Bachelor of Science Degree in Kinesiology from the University of Guelph, where she developed a strong foundation in health sciences and patient care. Her long-term goal is to become a Registered Nurse (RN), a testament to her commitment to the field.
In her current role, Maddy loves the opportunity to build meaningful relationships with both staff and clients. Her positive attitude, keen attention to detail, and mission-driven approach make her an invaluable member of the Home Instead team. She excels in multitasking and wears many hats within the organization, always ready to step in and support where needed. Outside of her professional life, Maddy enjoys a variety of activities that keep her energized and inspired. She is an avid fan of country music and loves attending concerts. Staying fit and active is important to her, and she enjoys spending time outdoors, whether it's hiking, running, or simply exploring nature. Maddy also values quality time with her family and friends, and she loves to travel, always looking for new adventures and experiences.
